Parental Consent
Here is a link to the consent form that Kinney Drugs needs to have on file. We will have forms with us that parents/guardians/authorized representatives can fill out that day or they may complete it in advance. It will need to be signed by a parent/guardian/authorized representative.
Follow-up appointments will be held at the same time as the first dose on the date of the second clinic, no need to schedule a second dose.
